At long last, Super Friday spring 2021 begins this week, Friday, January 21!
Attached you will find all of the important documents for this semester. Please take the time to read them carefully and let me know if you have any questions.
If you have not yet sent your Medical Liability Form, please do so BY WEDNESDAY NIGHT. You can scan and email it to me at dawnpruyn@homerunministries.com.
If I haven’t received your form by then, your students’ name tags will be pulled and you will need to turn your form in before they can head to class.
If you have not paid your full balance (or made arrangements with Anne Trapani for a payment plan), please do so before Friday in order for your students to participate.
ALL FAMILIES WHO DID NOT PARTICIPATE IN FALL 2020 NEED TO FOLLOW THE LINK BELOW AND COMPLETE THIS MEDICAL ISSUES, EXCEPTIONS AND BEHAVIORS FORM. If you attended in Fall 2020 and have no changes, you do not need to complete the form again.
